• A written & signed request is required for all Sacramental Records on file.  In order to protect the confidentiality of these records, certificates will only be issued to:

  • The individual named on the certificate
  • The parent or guardian of a minor child
  • A requesting parish or diocese

  
  Please complete the form in the left column to the fullest extent possible, then email, fax or drop off to the parish office.  Please allow 10 days for processing once received.
